Treatment strategy for warfarin over-anticoagulation
If a patient has a high INR consider the following possible causes
- New medication
- Concomitant illness
- Change in diet
- Change in activity
Ensure the patient is taking the correct dose
Select the appropriate patient category:
Patient with significant bleeding
Patient with INR above the therapeutic range at high risk of bleeding
Patient with INR above the therapeutic range but low risk of bleeding
